I got it to work in an odd way. The [event_form] page was a second child page. I moved it up a level to be a child page, and the form is showing up now. The odd bit it that the page is still showing in the second child page position it was originally in.
Any thoughts on how that could have occurred? I am a little concerned some cache refresh somewhere could cause an issue down the line.
Ok, it is definitely some sort of permalink issue. When I set permalinks to Plain, I can see the page correctly.
Again, the only problem I am having is when permalinks are set to Page name is that a page with a short code goes to a 404 once published, but not while the page is a draft.
I am able to publish everything else normally.
One thing I just did was try to publish a test page with the [event_form] short code as a top-level page and had no issue. It appears the problem is that when I assign the Add Event page as a child to parent page, there is a 404 issue. I am only having a 404 issue with child pages that have the [event_form] short code.
Now that I seem to have the issue better defined, what is the next step to troubleshoot?

I just went to Setting>Permalinks and clicked Save Changes as recommended by a google search to resave them. No change. I still get a 404. Let me know if I did the correct procedure. I am using Post Name permalinks, if that helps.
I previewed the page from the admin-dashboard before publishing and saw the add event form correctly displayed. However, once I published the page, I get a 404 when I go to the published page.
