No, unfortunately not. I’ve removed css, disabled all the plugins except Pirate Forms, as well as updated it to newest version (1.0.18) and nothing of that solved this problem.
I am far not a programmer, but I have a sense that Pirate forms interact with this theme somehow. I have no problem with forms in any other web page I manage, just in this particular. Unfortunately the owner of page does not want to change the theme 🙁
I’ve created a non-listed testpage with Pirate Forms and it behaves exactly the same:

Happens to be only on Firefox, desktop and mobile versions. IE, Chrome and Safari seems to show it correctly.

Contact form is put in as menu widget.There is also custom CSS for alignment of fields, but it does not affect the problem IMHO:
