Your code does work as does the previous one I shared except that it throws that warning in the debug.log. Surprisingly the default post_type is only set to page. Why???

I can confirm this issue. Using Tutor 2.1.5, WooCommerce 7.1.0, WP 6.1.1.
The student gets access the moment they click on the “Place Order” button.
The WooCommerce Order Status is “Pending” and Tutor > Enrollment says: Status: “Approved”.
Who is giving this approval and on what basis? Feel free to share the hooks or the process. Happy to debug this. Lot’s getting free access.

You say that your site has malware but there’s no confirmation how broken (or intact) your website is. A simple JavaScript or invalid markup issue can break plugins. If you want help with malware, this is not the forum for it.
