I found the documentation a little confusing around this. I didn’t really do anything special to the form, I just created one, so it must now be in the ‘published as a web page’ state it was talking about. From there I created the form entry in the wordpress plugin, then added the shortcode link for it into my page – so along the lines of [wpgform id=’4296′] and viewed that page.
One thing that tripped me up for a while is that when I was viewing my form in the Google Forms plugin area, it provided a Permalink to a page, but this never worked for me, kept on giving me a 404.

The plugin is this one: https://ww.wp.xz.cn/plugins/wpgform/. I’ve been able to get the form displaying on the page, but the problem has been the styling of the form isn’t using the default styles which you can see on the plugin page.
I’ve done a bit of digging and are not sure where the problem lies (I’ve tried disabling other plugins with no luck), but in this case, the form which is being output on the page is not using the right markup. For example, there are many classes being output along the lines of ‘freebirdFormviewerViewItemList’. I’m interested to see what you see if you run local tests.
