Having the same problem.
Tried following directions in the forum but same thing. Remove the JavaScript code but problem persists.

I deregistered all the scripts and styles from my theme functions but activity form still does not work.

Simplemaps still doesn’t load when WPTOUCH redirect landing page option is on.
Here are the things i’ve tried:
– changed desktop theme – nogo
– created custom page template – nogo
– removed all plugins except wptouch and simplemap – nogo
– tried headcleaner, minify plugins – nogoi have very little knowledge in programming. pls help
