A bit of custom CSS should most likely do the trick. What is the URL to your form?

Thanks, I’ve updated it and the date functionality is working, but now when I add a new text widget, the dropdown on the widget to add the text is not showing up. It looks like this: http://www.diigo.com/item/image/1rgjd/gv1s
When I disable the plugin, it appears again.
