Hi Fransing,
For me the issue was in the custom code. Check your custom css code. I am sorry I can´t remember now which piece of code was causing it; but I removed all my css code and started with a blank page and added each piece of code one by one until I found the offending css and then removed it.

I managed to find the problem and it was to do with a piece of css hiding the “Are you sure you want to log out? Confirm and log out” I removed this code and it is now working.
