UPDATE: I’ve fixed the first issue – it was a plugin conflict with Theme My Login.

Any advice on the 2nd issue would still be appreciated.

Hi Rui,

I tried disabling all other plugins (and updating this plugin to the newest version) but that hasn’t worked.
Any other ideas why this might be happening?
