Thank you for reaching out with your query. Currently, resetting the Lesson Complete Flag if a student fails the quiz after the lesson is not possible within Tutor LMS.
However, you can enforce a requirement for students to pass the quiz before accessing the next lesson. This setting ensures that students must successfully complete the quiz before progressing to the next lesson, thus achieving a similar outcome to what you’re seeking.

Certainly, implementing a requirement for tutors or admins to confirm a student’s performance before completing a course and receiving a certificate is not currently supported within Tutor LMS.

The header menu is part of the TutorStarter theme, it’s a theme depended feature. So you have to customize your theme header to add a menu like TutorStarter.
