Hi,
I’m having almost just the same problem.
The extra information does not appear during check-out, nor in the customer’s profile.
In my case, it also does not appear on the order screen on the woocommerce dashboard. SKU appears insteadThe settings are all unchecked in Advanced Settings:
Hide custom fields in Cart Page
Hide custom fields in Checkout page

Thanks to your first answer we did finally find the problem..
The site is currently under construction and because of that we have a index.php applied to prevent people from seeing the site. That is what is preventing the css from working. We did tried the calendar without the index.php and the colours and style showed as they have to..
It was our fault all the time..
