Nevermind I found you have to customize the menu under WP Settings > TinyMCE Advanced…
I am experiencing this every day randomly editing pages or media in the backend of word press. I find not hammering the database with changes to the same page too fast seems to help…? I can find no clear answer on the web, although many people are talking about it – no solutions. I can close that particular offending browser page and wait for some time, or I can close the browser completely and it works again immediately. Maybe some kind of timeout is happening on the server with the browser session? Once one browser page that is open has the error, then other pages open in the same browser will fail also. HOW it initially becomes a problem I don’t know. Has anyone nailed this down?
