I a sorry to read about your issue and sorry for the delay reply. This is one of our latest extensions and indeed, we forgot to subscribe to the support forum, so we were not notified about this ticket.
We’d be more than happy to assist you. What version of WordPress, PHP, WP Activity Log and this plugin are you running please?
Before we do any tests, can you check and confirm that you have the latest version of this extension and the main plugin, WP Activity Log?

Hello @robert681,
I had removed the plugin as I got no response. I will reinstall and we can move forward.
I am running WordPress 6.0.1 and PHP 8.1.0. The Activity Log is version 4.4.2.1 and the WPForms Log was the latest version before I deleted it.
After it is reinstalled I will send you the latest Debug Log info.

Thank you for giving the plugin another shot. Yes please, send us the debug log file and also the system information file (from the Help & Contact Us section in the plugin, System info tab.

I checked my logs again today and the error is no longer there.
Should I remove the temp plugin you sent me and reinstall the original file from WordPress?
Or should I continue to use the temp plugin as the replacement?

Hello @robert681,
In response to our discussion quoted below, I am not seeing the error in the debug log on my site’s server and I am not seeing any faults on the admin page or front end. The only place I saw this fatal error now is in the WPOptimize report I sent you. I am not sure how or when this is triggered so I cannot give you more advice. Sorry.

WPOptomize could very well be a key player here so thank you very much for providing this information. Can I ask, do you see this error during specific activity, such as when a form is submitted, or does it just appear to happen at random?

Hello @robert681,
There is no fault found in the debug logs and I do not have any challenges with the site. I do not think there is anything to check other than compatibility with WPOptimize as I only see this reported in their log but not in mine.
I am going to close the ticket as resolved for now and if it comes back at any time I will let you know. At that point we can spend time trying to trace this ghost.
