Sorry for the delay in getting back.
Well diagnosed this was a database issue not a WordFence issue I had other site problems as well.
All fixed, thank you for your fast response and putting me in the right direction.

In reply to: [Google Forms] Captcha is doubledMike, thank you for your quick reply. I am very sorry I had updated plugins and WordPress however for some reason I was still running v0.82 of Google Forms. I have just updated to 0.84 and turned CAPTCHA back on and I only get it displaying once.

In reply to: [Google Forms] CSS: Question SpacingThe first 7 questions, particularly Title, First Name, Last Name seem to a bit far apart but the numbered questions there after could be spaced out further.
I have no idea how to do what you suggest in the second paragraph, but in short if you have any ideas on how to change the CSS to improve the readability of this questionnaire.

Deactivated and Deleted v1.5.8.1 and installed v1.5.7.2. All my forms, old and new are working with tags again.
