For example, if a question has 3 options, I’d like the 3rd to be “Other” and have a text field that allows the user to enter text.

Thanks but no. My client wants the ability to receive the form submission in his email then reply (using his email client) to the prospect that submitted the form submission. In other words, reply to the form submission email much like we reply to a regular email.
