I am currently experiencing the exact same thing on my multisite setup. We also use Cozmoslabs Paid Member subscriptions and Profile builder. I know that PMS also creates roles, so not sure if there is a clash there somewhere, but we also have members installed on our multisite setup and experiencing the exact same error doing the exact same action.
