Could be absence of a firewall, yes? It’s a little confusing because a related site on the same server, without a firewall, has the red x for firewall but no notification flag in the WP menu.Thank you very much for the detailed information.
The good news is your reply got me looking again at the caching exclusions and, in the process, I discovered that the slug for the active donation confirmation page was /donation-confirmation-2, not /donation-confirmation. The former developer must have created a second confirmation page and deleted the original. So the confirmation was not being excluded from server caching. Once the “-2” was removed from the confirmation page slug the confirmation worked.
A lot of time and trouble caused by the simple slug issue. Your reply led me to look at it so thank you!

2.0.2 fixes the issues I initially reported at the beginning of this thread:
— the CC contact lists now display in the plugin,
— I can select a list for a form to populate, and
— account info (name + email) now displays in the ‘Disconnect’ screen.
Does 2.0.2 also include the vulnerability fix that was part of 1.14.1?
