Here are my answers to your questions. Thank you for your help.

Do you get the issue with all the forms or only with one?
The problem is only with this one form and only some of the submissions. Out of 35 submissions there were 7 that had an issue. I have a similar form set up which has had no issues.

Do you have the issue with uploading any type of file or only images files?
I listed all file types are acceptable in the form options.

What is the size of images that are not getting saved?
I do not know what size they are because they never get saved to the database or attach to the email. I ask for images not exceeding 2MB be uploaded.

Are there any other characters than numbers and letters in the uploaded file names?
I cannot tell. With the problem submissions, there are 5 images being uploaded per form and only one gets saved and a second with the same name and (1) after the name (eg. imageA.jpg, imageB(1).jpg)

What version of form maker do you use? Please check in the plugins section.
I have Form Maker Version 2.7.38 installed.
