I found the solution:
From the profile view you must select the cog on the right and select “Edit profile” this will then show all the customised fields. Only the fields that the user enters information will then show on the main profile section.

I had the opposite problem where I wanted my customised profile form only of a specific page but it overrode all the profile pages to my customised one.
You need to check the redirects.
If the role does not have permission to see that page then it redirects to the home page.
Just go through all the setting carefully and check if the form has global settings or customised setting. Also the page has a UM section that can overwrite this setting.
I am having the exact same issue and so do a lot of people now. Must be a big bug maybe because of the wordpress update.
I have tried everything including a 100% clean re-install and checked for conflicts.

That is the issue. The profile form fields are missing and that is the default profile form.
On the forms on the backend it shows the field when you click on live preview but on the front end it shows nothing. Just an unhappy face…
Sometimes on the custom form fields it wont show on the live preview but does show on the frontend.
Very weird… What could be causing this??
I am having the same issue, and did the conflict resolution and everything, re-installed everything twice!

I have done conflict test, I have done a clean re-install of everything, I am using twenty-sixteen deleted every other plugin and only have UM installed.
It is as clean as possible, not even one single post or setting change.
