Thanks for the response. Because we have pending litigation for ADA compliance, I have a couple questions:
1. Is this possible using radio buttons instead?
2. Would it be possible to implement an alternative solution, like wrapping the checkboxes in a <div> and giving that <div> an aria role=”group” with an aria-label?
3. Where in the codebase can I affect this change on my own?

I’ve tried your recommendations (delete plugin and reinstall) and also cleared all caches. Still seeing the same issue at the url above. Here’s the error I see in Firefox Console:

Hmm – while it’s nice to create leads, I’m specifically looking for a way to gather newsletter subscribers in salesforce/pardot and put the subscribers into their interested lists. Maybe this plugin isn’t ideal for that purpose? Thanks for your help – I would have thought that much like mailchimp or constant contact, there would be a similar plugin for salesforce. Any recommendations are appreciated!

I do have an export of my theme options from the original host prior to migration. I wonder if I imported that json export, would it correct the issue?

It looks like the site uses DP Evolve 2.0 and latest version is 3.6.2!!
