Here is the answer. Apparently, you have to input all the contact fields in order to submit the form. The contact fields were listed as optional.
Because of the individual’s circumstances, I do not think I can bother this man. He is under too much stress.

FONTS There is one more question about fonts – I looked at the support forum and entered in the font information on the quiz style sheet and it did not seem to work. Is that the right place? Of course, I might be inputting the wrong font code.
