I have exactly the same error since some weeks.
With twenty-eleven template also the names are wrong !!!
All the pictures uploaded have the same name and the same date.

I have not found how to fix it 🙁

I have the same problem too WP 3.3.2 and Nextgen-Gallery 1.9.3
