I ended up fixing the problem by manually deleting the unnecessary code directly from the WordPress database itself. It would be very helpful if you could give your users the ability to optionally remove the plugin data when they uninstall it. That would resolve this issue in the future.

I accidentally pasted the entire MailChimp form code rather than just the form action URL. Now I cannot remove it. Deleting and reinstalling the plugin does not even work, so I assume that editing the plugin files is not the answer. What files do I need to scrub to start over with a completely fresh installation?
