I am sorry about the trouble.
Please check the screen-options on the top right corner of the page. Make sure the concern metaboxes are checked.
Also, try disabling all other plugins for temporary time and check if other plugins are creating any conflict. Try changing to a default theme provided by WP.

Pro Version allows to integrate a UM form in your custom tab.Hope it will help you.
