And if you uncheck for not using the autocomplete input? What happens in that case? You can BTW put the meta box back in the navigation on the right.
I played a bit with my installations and I think you should check two things. First, is the meta box at the bottom of the editor and not on the right navigation. Second, please check if you use the autocomplete inputs as described here: http://msls.co/user-docs/plugin-configuration.html#advanced-settings
I will try to reproduce your problem during the weekend. Please provide as much as possible your configuration.

Automatically no. You could probably script something that could pair those pages.
The x-default for a page is the page itself if there are no translations available. At least as far I understood it… MSLS does that already
