Same problem here, too.
I don’t know exactly when this stopped working, or what current version was installed, but the fixes I’ve seen mentioned (downloading new copy from git) haven’t worked.
I can’t create or edit existing forms, nor can I view submissions. It does show a correct count of submissions, and I can export them and parse the content of submissions from the xml file, but that sucks.
