For proper working, you will need to add shipping methods with cities under booster add on shipping methods by cities/postcodes.

Yes, it will format address fields based on base country of your shop. For example if you are using United States as your base country it will format address as per US address format.

We have divided our plugins in various module add-on for different features. So as we went ahead improving new feature, we have created a different module. We are providing enable disable the feature for each one of them.
User is allowed to use a particular feature he/she wants to use. Regarding Slowing down Woocommerce we actually not much bother core functionalities and our modules have been build on standard hooks and action setup which will keep boosting Woocommerce.
Regarding Invoice, unfortunately, we currently support only PDF version of the invoice, packaging slip etc.
