That plugin began failing after the introduction of WordPress 4.4 and may explain the errors you are experiencing. The Unique Headers plugin does not do anything directly with custom database tables, only the ones provided within WordPress.

In reply to: [Unique Headers] qTranslate‑X and Unique HeaderFrom what you wrote, it seems that QTranslate does not use a separate post for each language. This seems like a flawed approach to me. If the plugin used a separate post, problems like this would not occur, since each post could have it’s own custom fields, featured images, custom headers etc.
I think in this situation, it would be best to custom build a plugin for adding custom headers for QTranslate. One could be made, which extended the Unique Headers plugin, or it could be an entirely new one.
