the best way is to enable the “Change order” option, this way the system will prompt only the available dates/times according to the user service/assistant selection.

Hi,
the alignment of the booking form inside the page can be controlled trough your theme settings / style.
To override the default style of our booking form, you can simply duplicate the CSS rule you want to modify and paste it inside your current theme style file.
