first of all, please verify that this is not only plugin based and other mails at your WP are going thru.

After reviewing the issue it says for both that the problem is caused by GMAIL settings:
The “From header sender domain not verified” error means you’re trying to send an email from an email address that hasn’t been verified as part of your account. You need to verify either the domain (like gmail.com) or the specific sender email address to be allowed to send, according to SMTP2GO

check the “from” in the settings of the form please!

1. Verify the Domain:

• Go to your email provider’s settings (e.g., Google Workspace, Sendinblue, etc.) and locate the “Verified Senders” or “Sender Domains” section.
• Add your domain (e.g., example.com) and follow the instructions to verify it. This usually involves adding a specific DNS record to your domain’s DNS settings.
• Once verified, the domain will be authorized for sending emails from that domain.

2. Verify the Sender Email Address (if domain verification is not possible):

• If you can’t verify the domain, you can verify individual sender email addresses.
• This involves adding the specific email address to the “Verified Senders” section and following the verification steps (usually a verification email sent to that address).

after testing i can recommend to remove the file upload and try again.

The file upload is something we are working on and only a very few users are using it.
The file will only transmitted with the mail and not stored somewhere else.
It must be stored temporarily at a folder from the plugin. make sure that the folder is writeable.

The field is not good for transmitting big data and each file must meet the wordpress gudelines of your mediathek library settings.
We will expand with a new upload element as soon as we find a proper solution.
