To help us further investigate the issue, could you provide a few more details. As far as I understand, the issue occurs once you try to login to your WordPress backend while the frontend is working as expected. I would like to clarify that we have recently released a new plugin version, so please specify the exact plugin version you are working with.
Please, also explain on which PHP versions you have made your tests and whether you have tried to disable all other plugins in order to eliminate any filtering of the headers earlier in the execution chain. If possible, please share the exact stack trace from your php_errorlog. We’d like to see the full execution chain leading to that Settings.php call.
You mentioned this is specific to certain Plesk setups. Do you happen to know if there is a specific Nginx/Proxy configuration or another host specific plugin active that might be duplicating these headers?

Combining JavaScript files may cause issues with scripts that require certain order of execution. You can leave the particular setting disabled and still benefit from the rest of the features offered by our plugin.

I did not manage to replicate the mentioned issue using a fresh WordPress application. In this case, you can try to reset the two-factor authentication on your end:

If the issue persists, you can also submit a support ticket via your Client Area at Siteground.

Our plugin is compatible with multiple hosting environments but there might be certain restrictions depending on the used software. We are not fully aware of the mentioned stack but it seems to be based on Nginx.
In case, you have experienced a particular issue on your end, feel free to provide us with more information about the exact error as well as with the logged entries in your error log.

I was not able to replicate the issue on a fresh WordPress application, so the error is most likely caused by plugin conflict or application configuration.
However, I would recommend you to try tweaking the frontend optimization options in our plugin and to check if there will be any difference:

If this is the case, you can leave the particular setting disabled and still benefit from the rest of the features offered by our plugin.

If you refer to the “Unknown Activity” tab, there you can find a list of all recent unregistered traffic with timestamp and IP information, along with the response code each traffic request resulted in. Generally, it tracks every HTTP status code, including 200 OK request.

The PHP warning might be generated by another plugin’s script execution. Since we can not further check the case on our end, I would recommend you to disable all your plugins one by one in order to determine the exact plugin.
Have in mind that the mentioned warning is not a fatal PHP error and it can be safely disregarded.
